Jewish Senior Services offers care navigation services to address needs that may arise as one ages, and supports those seeking guidance on aging issues.

An ElderCare Navigator Can:

  • Provide coordination of resources, especially during care and health transitions
  • Make home visits to assess home safety and health needs
  • Evaluate in-home care needs
  • Arrange in-home care and select care personnel
  • Assist with other living arrangements
  • Provide consultation and advocacy in dealing with providers, benefits and services
  • Coordinate medical services and appointment
  • Address emotional concerns and provide supportive counseling
  • Provide long-term planning services such as assistance with long-term care directives, living will, health proxy and powers of attorney
  • Communicates and coordinates with family members
